  • No. We are 100% referral based, meaning you need to talk with your PCP about a referral. They have a form to send us so that we know more about why you need to be seen.

  • Yes, we do accept it. There is a process for receiving a discount for cash pay. Please call to find out more.

  • Jennifer is contracted as in-network with most private insurance plans.

  • Jennifer is credentialed with Oregon Medicaid open card and individually with JCC and AllCare.

  • Yes. There is an application process with explanation that you can come in and complete once accepted as a patient.

We try to respond to all inquiries within 24 hours. But since we are also actively providing care for established families, it could take up to 5 business days to receive a response. If you have not heard from us within 5 business days, please call the office.
